Order Form

Follow these steps to order an LCD screen from ScreenTek:

1. Check availability.
   Call, email, or Skype us to make sure we currently have the exact screen you need. Our website lists all screens we sell, and does not reflect actual daily inventory changes.
2. Read our Terms & Conditions
3. Print order form.
   Download, fill out **, print, and sign our fax order form, in PDF format *.
   If sending us your laptop, you must also fill out page 3 of the form (ScreenTek Packing List) as per Section 4.3 of our Terms & Conditions.
4. Fax it in!
   Send the form in to us via our fax numbers.

* To view & print PDF documents usually requires Adobe Acrobat reader or similar software.
 
** If your software is up-to-date (Acrobat / Reader 7.0 or higher), you'll be able to type in the form to fill it out; otherwise, you can print it and fill it in by hand, or update your Adobe Reader software (so that the form can automatically calculate certain fields when you type in it).
